Our Telehealth services, powered by SesameCare, allow quick access to care without needing to visit a clinic. If your provider determines additional follow-up is needed, a referral will be provided.

Available Services Include:

• General Illness & Urgent Concerns: Cold and flu symptoms, sore throat, UTIs, skin or eye infections, Covid-related care, and more.
• Medication Management: Thorough review of prescriptions, side effects, and refills for conditions like high blood pressure, diabetes, or birth control (excludes controlled substances).
• COVID-19 Symptom Review: Assessment of symptoms to determine if in-person testing or further care is recommended.

All Telehealth visits are conducted by Cynthia Fagundes, FNP, a board-certified family nurse practitioner with over 12 years of experience in family, adult, and geriatric medicine. Whether you’re checking in on a chronic condition or feeling unwell, Cindy provides compassionate, convenient care through secure video visits.
