Telehealth

Our Telehealth services provide convenient access to care without needing to visit the office. Virtual appointments allow patients to connect with a provider from the comfort of home for select medical concerns and follow-up care.

Available services may include:

• General illness and urgent concerns
• Cold and flu symptoms
• Sore throat evaluations
• UTI concerns
• Skin and eye infections
• COVID-related symptom review
• Medication management
• Prescription refills for select conditions
• Blood pressure and diabetes follow-up
• Birth control management
• Wellness follow-up appointments

If your provider determines additional follow-up or in-person care is needed, a referral will be provided.

All Telehealth visits are conducted by Cynthia Fagundes, a board-certified Family Nurse Practitioner with more than 12 years of experience in family, adult, and geriatric medicine. Whether you are managing a chronic condition or addressing a new concern, our goal is to provide accessible and compassionate care when you need it most.
